Child Birth Certificate:
Scanned copy or statutory age declaration. Remember to bring original to assessment.
Parent / Guardian Photograph:
Passport photo of the applying parent or authorized guardian.
Previous Term Report Card:
Required for pupils transferring into Basic 1 through Senior Secondary (PDF or image).

No scanner? Clean phone camera photographs in good lighting are fully accepted.
